  • Over the phone, via email, or online through the www.munchkinnutrition@outlook.com website enquiry form when you are booking your consultation or enquiring about our services

  • An intake form completed at the start of your care

  • Dietetic consultations, which may occur in person, over the phone, or via telehealth

  • Referral letters from doctors, allied health professionals, or other relevant referring bodies

  • Handover from other health professionals involved in the clients care and relevant to ongoing services to be provided by Munchkin Nutrition and Dietetics

All personal information collected during consultations is securely stored in Cliniko, a secure practice management software. Cliniko stores all patient information safely on secure servers, ensuring that your data is protected and only accessible by authorized Munchkin Nutrition and Dietetic staff.

 Enquiries through the website are received as an email to our business email address and followed up by our staff accordingly. This information contains your contact details as per your submission and is used to contact you and provide you with information about your enquiry. We do not share this information with any other services or third party providers.
